Taylor Swift and Dakota Johnson are seemingly two friends bonding over their mutual toxic ex in the “Patient Zero” music video … until the twist ending.
The pop icon released new single “Patient Zero” from “The Life of a Showgirl: The Encore” on Sept. 25. On the mid-tempo track, Swift sings about being the first woman to fall for a man with “the devil on his shoulder under his halo.” When he moves on to the next woman and also treats her poorly, Swift offers her a shoulder to lean on.
Swift debuted the “Patient Zero” music video at the 2026 Video Music Awards on Sunday, Sept. 27. In the music video, which she teased on her Instagram prior to its release, Johnson takes on the role of the latest woman scorned by a seemingly charming man, played by Colin Farrell.
The music video opens with the singer joining Johnson in a graveyard. The two then live parallel lives as Swift helps a physically ill Johnson recover during her unhealthy relationship with Farrell’s character.
Throughout the video, Johnson and Swift wear the same outfits, with the singer watching close by while Johnson struggles in the relationship. The video also cuts to flashbacks of Swift having a similarly harmful with Farrell. Model and Cara Delevingne makes a cameo as a woman Farrell flirts with at a party.
After the bridge, the music stops as Johnson and Farrell have a manipulative conversation during their card ride home. Farrell chastises Johnson for not being different from “her.”
The video ends with a surprise reveal: all the footage of Swift and Farrell happened before he poisoned and murdered the singer. Swift’s character is a ghost looking after Johnson, writing a warning in blood for her to “run.”
In the final seconds, Johnson leaves flowers at Swift’s grave. Her tombstone reads: “Beloved wife and daughter gone too soon.”
Swift directed “Patient Zero” and worked with Oscar-winning cinematographer Emmanuel Lubezki.
She shared more details about the music video in a lengthy post on Instagram.
“Getting to work with @chivexp was something out of a dream sequence, and I learned so much from getting to watch his genius in action,” she gushed before thanking her co-stars.
“Giggling with your friend because you’re twinning for an entire shoot was a new and truly delightful experience). And Colin’s turn as a villain is even more impressive when you get to be around him. He’s an absolute light and as kind as they come. @caradelevingne, thank you for agreeing to play the lusty other woman. That’s what friends are for?” she joked.
Swift said that the music video allowed her to explore different genres, such as gothic ghost stories, true crime accounts and psychological thrillers.
“The house was a metaphor for the coldness of a relationship that’s faded and atrophied,” she explained.
After thanking her production designer and editor, she concluded, “And lastly, thank you to the @vmas for giving us a World Premiere at the place where celebrating music videos became a thing. And so much love for you guys, always.”
Swift had a big night at the awards show as she was presented with the inaugural MTV VMA Artist Director Honors minutes before the video premiere. The new award recognized “groundbreaking music artists whose work behind the camera has expanded the possibilities of music video and advanced the art of visual storytelling.” She won video of the year for “The Fate of Ophelia.”
Before the arrival of the music video, Johnson spoke about working with Swift for her latest music release while on the red carpet at the London premiere of the actor’s film “Verity.”
“That was just the best. Working with your friends is like the most fun thing on the planet because we’ve known each other for so long,” she shared in a clip posted on X. “We really respect each other’s creative minds, and to be able to work alongside her and support and assist and be just a person that she tells what to do is wonderful.”
“Patient Zero” is one of four new songs on the extended version of Swift’s “The Life of a Showgirl” album.
Swift told fans she decided to pen the new tracks after she saw how much they supported her record-breaking 12th album when it was released last year.
“I took a trip to Sweden to celebrate with Max and Shellback, my two collaborators on this album. Really we just wanted to reflect and take in how grateful we were for the moment we were in, but there was a studio there and we did what we do when we feel anything at all: We wrote more songs,” she explained on Instagram. “I hope you love them, because they were born out of pure gratitude for your exuberance for and celebration of the album we made.”
The 2026 MTV Video Music Awards are being hosted by Snoop Dogg at the Peacock Theater in Los Angeles. Madonna leads with 11 nominations. The show will feature special presentations, like the MTV VMA Artist Director Honors and Video Vanguard Award. Taylor Swift, who received nine nominations, will receive the Artist Director Honors award and debut the music video for her new single “Patient Zero.” The ceremony will include tributes to Dolly Parton and George Michael, as well.
